Secondary breast procedures

The need to remove an implant does not mean that your breasts will necessarily become misshapen or saggy. Plastic surgery procedures can restore a beautiful appearance to your breasts - or even enhance it. Once the implants are removed, the stretched skin will need to be tightened, and the nipple and breast tissue lifted - in other words, a mastopexy.(breast lift) is required. If you also wish to maintain a larger breast size, the space left by the implant can be filled using a fat graft.

Contrary to popular belief, breast implants do not need to be replaced. Only the types of deformity discussed in the previous chapter constitute a formal medical indication for implant replacement. However, if you wish to significantly alter the shape or size of your breasts, then replacing the implants or undergoing breast contouring with fat grafting may be the right solution for you. Implant manufacturing technology is constantly evolving.

Changes are being made to their composition, outer shell, and weight. Many of the issues associated with older-generation implants are unlikely to occur with the latest models. Furthermore, all reputable manufacturers provide a lifetime warranty against shell rupture, as well as a long-term warranty covering implant deformity caused by the overgrowth of the connective tissue capsule (capsular contracture).

It may happen that, following the insertion of implants, the skin and breast tissue stretch and shift downward, resulting in what is known as a "waterfall deformity." If the implants are correctly positioned and remain intact, a breast lift performed while retaining the existing implants can be an excellent surgical option. In such cases, I perform a breast lift by carefully dissecting the tissues along the implant capsule, allowing patients to enjoy a refreshed breast appearance without the need to remove or replace the implants.

Fat is a tissue that contains a large number of stem cells, making it well-suited for autologous transplantation (autografting). These stem cells stimulate neoangiogenesis - the growth of new blood vessels - which provide nourishment to the transplanted fat. Only when the transplanted fat cells are adequately nourished by these new blood vessels can we say that the fat has "taken" (successfully integrated).

Fat can be grafted under the skin or beneath a muscle. It is an excellent method for correcting irregularities or filling volume deficits in the breasts. It is most frequently used as a corrective procedure - for instance, following oncological surgery, the excision of breast lesions, or to address aesthetic complications resulting from a breast lift, breast reduction, or complex breast augmentation with implants.

Fat grafting can also be a primary procedure; if liposuction is being performed on another part of the body, the harvested fat cells - rather than being discarded - can be grafted into the breasts, for example, to achieve an augmentation effect.
